Re: COVID-19 Vaccination Program – Registration for Seniors
As you may know, the Covid-19 Vaccination Program – Phase I is in progress, and as per Ottawa Public Health (OPH), Phase 2 Vaccination is likely to commence soon, probably in March 2021.
The City of Ottawa and OPH are seeking help from community organizations to identify a list of individuals who will be eligible for Phase 2 Vaccinations. ICCC Seniors program is taking this initiative to be ready to help the City of Ottawa to carry out the Phase 2 Vaccinations.
OUR APPEAL TO YOU: Kindly provide us your information by completing the short Registration Questionnaire below:
- If you are above the age of 80 years, please click here.
- If you are above the age of 70 years and have severe underlying health conditions, please click here.
- For a list of underlying health conditions, please click: Certain Medical Conditions and Risk for Severe COVID-19 Illness | CDC
- If you would like to volunteer to drive the seniors for vaccinations, please click here.
FYI, as per OPH, persons who are the caregivers will also be vaccinated along with the eligible seniors.
Please note that ICCC is planning, for the convenience of our community, to set up an easily accessible Clinic facility for vaccinations. For additional information, please contact Mr. Anil Sukhija (613-791-7593) – chair-seniors@copy.icccottawa.org.
We are all deeply indebted to all the dedicated medical and healthcare professionals for their untiring service in dealing with this pandemic.
As always, ICCC is open to your suggestions and feedback to better serve the community needs. Thank you very much for your cooperation.
Kind Regards.
